still allow people to override them in defaults.yml

use the world-wide pool by default, but specify north-america in
user.yml. Also, documentation. This way Sovereign will still behave the
same, but the NTP servers can be changed when desired.

Remove all configuration for MySQL and configure PostgreSQL as the main
database.

All *_mysql_* options have been changed to *_db_* options.

Postgres requires the database user to have a password in order to
connect via localhost. The db_admin_password option is used to set the
password of the admin user (usually postgres).

Added friendly_networks variable to denote whitelisted networks

correctly. The number of days that a certificate will be considered
valid is now a user-controlled variable and is set to five years by
default. (Fixes Issue #87)

That way the default values will not be applied by mistake. The user
needs to uncomment the lines from `vars/users.yml` and set them.

That way we can override all the config from a central location. The
`user.yml` file overrides `defaults.yml`. `user.yml` was added to
`.gitignore` in order to allow a user to change it without having to
worry about merges. It's easy to do a diff to see what changed between
the defaults and your own settings.
